The Single Point of Access Service which will incorporate the Virtual Ward is continuing to develop and change in order to meet the needs of the local population. The service is undergoing expansion and we now require Band 3 Service Advisors. The hours are varied and include a mixture of shift patterns to support a 7 day, 24 hour service. Night shifts will be an integral part of the role.

This post would suit candidates who have previously had "hands on" patient contact and who also have sound administration skills and are confident utilising electronic patient records. Experience with telephone triage would be desirable but not essential. These posts will suit individuals who enjoy working within a developing service and are motivated and enthusiastic.

The posts could be a mix of full-time and part-time staff and if you have a preference for the number of hours you would wish to work please state this in your application.

The main responsibilities are:

  • To provide an effective single point of referral and call handling service for the community services provided by NLaG.
  • To make supported decisions on the most appropriate care pathway and direct service users/professionals into the appropriate services area ensuring equality of access across a range of services.
  • To carry out back office functions as required relating to the timely processing of referrals and to ensure patient records are accurately and quickly updated.
  • To provide co-ordination of visits and monitoring of the referrals for NLaG Unscheduled Care Team home visiting Practitioners 24hrs per day 365 days per year to support 24hr Clinical Assessment Service.
  • To work closely with and supported by the Single Point of Access nurses and Unscheduled Care Team

The SPA service aims to:

  • Offer an alternative to secondary care where safe and appropriate
  • Assist in the reduction of the length of a hospital stay by ensuring patients are signposted to the most appropriate place
  • Assist in the management of patients accessing the GP Out of Hours Service which includes receiving and managing referrals from NHS 111
  • Provide a telephone triage service and/or a secondary assessment for patients who have ongoing health needs and signpost to the most appropriate service or provide self-care advice

The virtual ward describes an enhanced package of healthcare provided with the aim to maintain care within the home wherever possible. It is an alternative pathway for those patients:

  • who do not require acute medical in-patient support and are clinically fit for discharge, but do require on-going nursing interventions and care and/or intensive therapy
  • where admission to an acute hospital can be avoided through the provision of intensive therapy and/or nursing interventions and care
