About the Role
The project involves co-designing innovative and sustainable care pathways with the Alliance Partners and through codesign activities with the community and staff, and at the same time enrolling the community to the relevant primary care facilities. It is expected that the project will integrate with and leverage existing integration and care pathway projects operating across the Central West catchment, including the WQPHN Health Care home initiative.
The position of Project Manager -Transition Lead Project West will be responsible for ensuring the successful delivery of Project West's care pathways for the Alliance. This will require the ability to work closely with Queensland Treasury Corporation (QTC) as the key project coordinator and the Alliance Partners. It will also require you to be able to organise resources and data from Alliance partners to support the project.
The total monthly remuneration value of up to $11,570 p.m., is comprised of: salary between $4,346.10 - $4,664.30 p.f., (AO7); employer contribution to superannuation (up to 12.75%); annual leave loading (17.5%); Temporary full-time; Accommodation assistance available (conditions apply); Applications will remain current for the duration of the vacancy.
