What You'll Be Doing

Child and Family Health Nurses (CFHN) working in the Sustaining NSW Families (SNF) program are the front-line service providers. The CFHN in SNF practices at an extended level of nursing, working in a primary health care model, undertaking sustained health home visiting (SHHV) with families who are identified as part of the SAFESTART process as having Level 2 vulnerabilities. The SHHV program works with these vulnerable women and their families, from the antenatal period, potentially until the child is 2 years of age.
 

The (CFHN) is responsible for:
Providing best practice evidence-based care to identified vulnerable families using the Sustaining NSW Families program model, Promoting parent-child relationships and optimal child development by working in partnership with parents, The case management of families to ensure they have an effective and integrated plan of care and working in partnership with general practitioners, midwives, perinatal mental health, government and non-government organisations and the members of the SNF multidisciplinary team.
