We are expanding our Hospice Services and are looking fordynamic healthcare professionals to join our Hospice Nursing Teams in our Home and Inpatient Unit Settings. Our RNs are part of an interdisciplinary team who specialize in symptom management of terminally ill patients. The ideal candidate will be flexible with availability to work between both the home and inpatient environments. Hospice experience is preferred but would consider a strong candidate with recent acute or critical care experience.

Responsibilities

The Hospice RN Case Manager utilizes the nursing process (assess, plan, implement, evaluate) to assume primary responsibility for the care of terminally ill patients and their caregivers in consultation with the primary physician, Hospice Medical Director and the Hospice interdisciplinary team. RN Case Manager evaluates need and coordinates care from other disciplines including Clinical Care Partners, Social Workers, Chaplains, Bereavement Coordinators and Volunteers. The hospice nurse provides physical and psychosocial support to the patient and supportive care to the

caregiver and family that will assist them in accepting and preparing for the death of the patient. RN Case Manager reports to the Hospice Nurse Manager. Use of personal vehicle required. Mileage reimbursed.

Joining the Home Hospice Team comes with some fabulous perks including:

  • Self-scheduling with the ability to fill open shifts with all 3 hospice departments (home team and both 18 bed inpatient hospice units located in Marietta and Austell)

  • Hospice RNs practice at a high level to manage patient symptoms with the goals of care focusing on maximizing comfort and peace at the patient’s end of life

  • Care is at an acute care level and care includes IVs (PCAs, cardiac drips), chest tubes, peritoneal drains, feeding tubes, etc.

  • Mileage and cell phone stipend reimbursements

This position will cover South Fulton/AMC territory for home hospice patients.

Qualifications

Required Minimum Education:

  • Graduate of an accredited school of nursing. Current Georgia license as a Registered Nurse.

Required Minimum licensure/certification:

  • Current Registered Nurse license in the state of Georgia.

  • BLS required.

  • CHPN certification preferred.

Required Minimum Experience:

  • Two (2) years of acute care or hospice/home care experience preferred.

Required Minimum Skills:

  • Possesses emotional maturity.

  • Possesses interpersonal skills.

  • Demonstrates excellent assessment and clinical judgment skills. Possesses knowledge of developmental characteristics (physical, psychosocial, intellectual) of people of all ages (neonate, infant, child, adolescent, adult, geriatric).

  • Possesses knowledge of disease processes affecting people of all ages.

  • Demonstrates effective written and verbal communication skills.

  • Demonstrates effective use of computers for patient care documentation.
